PS: It looks somehow like Casper's, but I had posted the skull idea some posts ago. Thanks Casper for the "No Smoking" text and for letting me use it on this entry. I didn't know what text to use, without it the work would not be complete

I'll tell you anyway!I am not even going to ask how you did it Blake. It looks great!

This is the story of this entry...
I originally had the idea of making a sign with a cigarette and smoke coming out from it converting into a skull. I looked for good smoke images to modify but I could not find what I was looking for. When I was giving up, I found this image:

The image is perfect, but if I had represented my original idea it would have been just copying and pasting. So the idea of the gun came to my mind, and when I read "Smoking Kills" I felt it was the perfect line for the design.
Some people may think I did nothing in this entry, but coming to that idea, and finding the perfect images was not so easy.
Anyway, I want to show how this effect may be done in Photoshop...
Let's take your first entry as an example Casper.
The image can't be modified a lot because it won't look real. So a smoke image with a shape similar to a skull is needed, your image is pretty good too. Details are made using the Liquify or/and Smudge Tool (liquify is better, but in certain cases smudging may be needed), some layers in Overlay mode (this is optional), eraser tool and brushing some highlights/shadows.
This way, something like this can be obtained (it was made fastly, but it's just to show that it can be done):
